Hints:

Information for patients:
  • In most cases appointments are scheduled to last for approximately 3½ hours (plus or minus half an hour).
  • You should be well-rested and have something to eat prior to the appointment.
  • If necessary, there is client parking in the building, entrance on the south side of the building (up the ramp).  The client should pay for four hours ($4), which will be reimbursed upon arrival (if this does not appear on the menu screen, choose "other options").  The client parking area will accommodate SUVs and similar size trucks, but not high pickup trucks.  There is also limited outdoor pay parking about 150 - 200 meters south, on either side of 14th Street, as well as free parking on the west side of 16A Street.  Parking on the street closer than that is by residence permit only or limited to two hours which is not enough time.

Documents requested from referral sources:
  • Ambulance records.
  • Hospital records including Discharge Summary, ICU Transfer Summary, emergency room notes, doctor’s progress notes, consultation reports, psychology reports, reports from any rehabilitation disciplines, results of imaging studies (MRI scans, CT scans, X-rays), and EEG results.
  • Professional consultation reports.
  • Results of any imaging studies, EEGs, or other investigations performed as an outpatient.
  • School records and academic transcripts if available.
  • Work performance evaluations if available.
  • Transcripts of examinations for discoveries or extracts therefrom if relevant.
  • Raw data (full test protocols) from any psychological or neuropsychological tests which have been performed. This information will have to be sent directly to me from the psychologist, who will require explicit written consent for release of this information.
  • Any other relevant information.
  • For combined neuropsychological/vocational evaluations: tax records (T1 forms or similar).
